Step by step instructions for getting licensed to fight at a state sanctioned event.
1. If you're fighting within a few weeks start ASAP because it can take a few weeks to get the bloodwork and drug test results back.
2. Get Bloodwork done to test for Hep B, Hep C and HIV - Most blood banks will actually pay you to donate blood and they usually test for the same diseases.
3. Click (HERE) to download the State Combatant Application and fill out pages (1) and (2).
4. Have a licensed physician give you a sports physical AND have him/her fill out page (3) of the application (IMPORTANT - The Idaho State Athletic Commission DOES NOT require the Cerebral Hemorrhaging question to be answered - No MRI Required.)
5. Take a drug test
6. Take the application to a public notary and have them notarize it on the bottom of page (2)
7. Once you receive the drug test and bloodwork results send it all with the notarized application and a $30 check for the states fee to: STATE ATHLETIC COMMISSION BUREAU OF OCCUPATIONAL LICENSES 700 West State Street, P.O. Box 83720 Boise, Idaho 83720-0063
